Deadline for issuing: 14 days
Validity of document/individual administrative act: indefinitely
Control authority for the authority providing the service: the head of the respective structural unit
Requirements, procedures, instructions
I. Submitting an application
The application is in free form text.
II. Determination of fee
The fee is determined based on the number of pages of the document, which will be prepared.
III. Receiving of the document
Upon payment of the determined fee the document is handed out to the applicant.

The service fees are determined in Tariff Nr. 4 of Fees Charged by the Ministry of Interior under the Law on State Fees.
